What you will do

  1. Describe the initiating event and associated top event.
  2. Record the hazardous phenomenon and its consequences.
  3. List passive and active risk-control measures and their effect on the final rating.

Pitfall to avoid. This rating uses the fixed PIGC scales from the hazard-study framework, which differ from generic severity/likelihood scales. Do not modify them; they are prescribed.
